The neck fat cells treated by neck liposuction are removed from your body and will never return. These neck fat cells will not be replaced by other neck fat cells. In my opinion, general anesthesia is superior to intravenous sedation in all respects for rhinoplasty surgery for one main reason-patient safety. With general anesthesia, a small breathing tube is inserted by the anesthesiologist and a balloon on the end of this tube is inflated. If the patient bleeds, the blood will travel down the back of the throat and can enter the stomach and lead to nausea and vomiting- or worse- wind up in the patients lungs and lead to a pneumonia. With intravenous sedation, the patients gag reflex is diminished by the sedative drugs. For these reasons, I prefer general anesthesia as compared to intravenous sedation for rhinoplasty surgery.
Where are the incisions made for blepharoplasty? Are there scars after eyelid surgery?
When performing blepharoplasty of the upper eyelid, Dr. Kole makes an imperceptible incision in the upper eyelid crease. These incisions are made in such a way that they will heal into a thin virtually hidden scars in the upper eyelid crease. During the eyelid surgery blepharoplasty of the lower eyelids, Dr. Kole makes a small incision on the inside of the eyelid, which results in no external scar whatsoever – the incision is ‘invisible’ or a small scar immediately under the lash line when there is extra skin and fat of the lower lid that needs to be removed.
Does insurance cover the cost of blepharoplasty?
If your blepharoplasty (eyelid surgery) is performed solely to achieve a cosmetic benefit, insurance will not provide coverage. In such instances where your vision is being impared by drooping of the upper eyelids, insurance may cover part of all of the cost of the procedure.In these cases a non invasive visual field study may need to be performed. Can I still get mammograms with breast implants?
Yes, the mammogram technician can perform a mammogram with breast implants in place. Please tell the technician that you have implants so they can protect the implants from damage from the mammogram machine and also do an additional study called a “displacement” view so more breast tissue can be visualized by the mammogram.
How can I tell if my breast implants rupture?
With saline implants, a ruptured breast implant will result in deflation of the implant, absorption of the sterile saline by your body and an obvious volume change in your breast size. The silicone outer shell that held the saline will remain and have to be removed and replaced with a new implant. There is no repair or “patch” kit for breast implants.
Silicone breast implant rupture of the older style will result in a subtle or obvious change in shape and feel of the implant and the breast. These implants when ruptured can be diagnosed by mammogram, where a “linguini sign” of the crumbled shell can be seen on X-Ray. The newer cohesive implants or the newsest highly cohesive “gummy bear” style 410 implants when ruptured will not have any discernable difference in breast size or shape. These new implants rupture can not be determined by x-ray or mammogram but must be diagnosed by MRI only.
Can I breast feed with breast implants?
Breast implants will not affect the ability to breast feed because the implants are usually placed under the chest wall muscle. There have been some studies of the old type of silicone implants (prior to 1992) having silicone particles in the breast milk after they have ruptured, but these implants are not in use any longer. The areolar incision may sever some milk ducts but this is usually not a probem with breast feeding.
How long is my breast augmentation surgery going to take?
The surgical procedure for a straight forward breast augmentation takes about 1 hour. The use of silicone implants adds only a few extra minutes of time for their placement. Patients with asymmetrical breasts requiring two different sized implants or patients requiring skin manipulation (breast lifts) to address symmetry issues require longer operative times.
Will I need help at home after my breast surgery and for how long?
Yes, although not mandatory, it is advisable to have a friend or family member assist you for the first day and night after your breast surgery. They can help you with meals, medication management and other household activities. You should limit your activities after either a breast reduction, breast augmentation or breast lift to reduce your chance of post operative bleeding and complications.
How long will I be in pain after breast surgery?
Although everyone is different, the most discomfort is experienced within the first day or so. With oral pain medicine, strict avoidance of arm activities, the pain will greatly diminish in 2 to 3 days. Breast augmentation is usually more painful than a breast lift because of the extra discomfort that occurs when placing the implants below the chest wall muscle. You should be back to normal in about a week and exercise can be started safely at two weeks.

The best candidate for a blepharoplasty (eyelid lift) is a physically healthy man who is realistic in their expectations and looking for improvement of their droopy upper eyelids and/ or bulging lower eyelid bags. A blepharoplasty will not correct sagging of the brows, or deep lines, wrinkles or furrows in the forehead. These patients may benefit from other procedures, such as a forehead lift (browlift) or skin resurfacing. At one weeks time, the sutures will be removed and you may return to normal activities. Will I lose sensation in my nipples after a breast reduction?
Dr. Kole performs breast reduction surgery using a technique that allows for the nipples and areola’s nerve endings to remain intact, which allows nipples to maintain full sensitivity. This is performed by preserving the midsection of the bottom of the breast and a pedicle of tissue to preserve both the nerves and blood vessels. However, a risk of this surgery is the permanent loss of sensation if the nipple needs to be severed during surgery and replaced as a free nipple graft. This is only performed in rare cases when the pedicle length is so long that the blood vessels and nerves to the areola will get kinked. How are breast lifts done?
There are several different breast lift techniques which vary based on how much skin needs to be removed. What are the types of breast lift techniques?
Concentric (doughnut) breast lift
This lift is for women whose breasts are smaller and not as severely sagging and when a crescent lift will not result in enough elevation. A doughnut-shaped piece of both areola and breast skin is removed from around the nipple areola complex such that they can be shifted upwards, and then the remaining skin is sutured around them in their new location. This results in a scar completely around the areola complex.
Lolly pop lift
A variation of the concentric breast lift where another additional incision running downward from the areola edge towards the breast crease is made. This removes more skin on the bottom half of the breast in addition to elevating the nipple areola complex. A strip of skin can be removed and the edges sutured together. With this type of lift, more firming of the breast is achieved.
Anchor-shaped breast lift
This technique is used for women with larger and more sagging breasts with a significant excess of breast skin. This is the most frequent technique used for breast lifts. In addition to the periareola and vertical incisions, another incision is placed in the breast fold. The final shape of all the incisions together looks like an anchor.
Is there a lot of pain after a breast lift?

How long does a breast lift take?

- The dressings will be removed in 24-48 hours. After that, Dr Kole will want you to wear a surgical bra for several weeks.
- After one week, the sutures will be removed
- Bruising will fade after several weeks, typically 2 to 4 weeks.
- In the first weeks or so you may feel some numbness around the nipples as the nerves in the area can be affected and take a few weeks to become less inflamed. You might also feel shooting pains or low-grade electric type shocks or spasms because the nerves were a bit traumatized by the procedure. These will also fade within a few weeks. The scars will fade over a period of months becoming paler, flatter and softer. The increased fullness and shapeliness of your breasts will be immediately apparent, but your breasts may take up to a year to fully settle into the new position.
Does breast lift surgery enlarge your breasts?
By lifting and firming up the breasts and removing excess breast skin, a breast lift (mastopexy) will make your breasts look larger. I have had excellent results with the modified brachioplasty procedure. In this operation, the scar is located in the hollow of the axilla (armpit) rather than being visible from the elbow to the axilla. There is no optimal age for facial surgery however one should consider facial surgery when the early signs of aging begin. This usually happens in the early 40’s and progresses steadily from there. Early signs of aging consist of worsening of the fat pockets of the lower lids, increasing wrinkles, deepening of the folds between the nose and the mouth and early jowl formation.
Can insurance cover an upper eyelid surgery?
If there is documented loss field of vision especially on upward gaze, then many insurance companies may cover cosmetic functional excision of the upper lid skin. This visual field study is usually done by an opthalmologist.
What is the difference between a brow lift and an upper eyelid lift?
A brow lift repositions the eyebrow in a more pleasing contour while an upper eyelid blepharoplasty removes the upper eyelid excess skin and deepens the eyelid fold and removes the overhanging skin.
Do I have to use an implant for chin augmentation?
No. In addition to synthetic implants, one can opt for a chin advancement using your own bone. This is called a sliding genioplasty. This procedure is performed by transecting the bone of the chin, moving it forward and securing it with titanium scres and a small plate.
How is a sliding genioplasty performed?
Through an intraoral incision, the mandible (chin bone) is cut on an angle and moved forward to give the chin more projection, then secured in place using a small titanium plate and screws.
How do I know if I need a brow lift?
If you have low arched brows and extra skin with deep folds of your upper eyelid, then you may be a candidate for a brow lift. A brow lift repositions both the upper eyelid in a more aesthetically pleasing shape and also removes some of the redundant upper eyelid skin at the same time.
What is otoplasty?
The optimal time to seek otoplasty is after a child’s ears have developed completely (normally around the age of five) but before the cartilage hardens and becomes less pliable. While otoplasty is considered a safe procedure that is often successful, complications including bleeding, infection, recurrence, and overcorrection are possible. Recovery is often fast, though patients are asked to wear protective wraps for several weeks following the procedure. Otoplasty does not improve or harm a patient’s ability to hear.

Do you have to remove your belly button when you have a tummy tuck?
Usually not. In a fully tummy tuck, the umbilicus (bellybutton) is left attached to the body and the skin around the bellybutton is removed and pulled down to achieve tight abdominal contour. In certain cases, of a mini tummy tuck, the bellybutton is excised from the skin and pulled slightly downward in a slightly lower position to remove the small residual laxity of the upper abdominal skin. The slightly lower position of the umbilicus is not noticeable.
How can I tell if I need a chin implant?
The general rule is that a person would benefit from a chin implant when a vertical line is dropped down from the upper lip and the most projecting portion of the chin is more than 2mm behind this vertical line.

Yes, all surgery leaves scars, some more visible than others. Transaxillary incisions are made through the armpit approach and scars may be noticeable when you lift up your arms. If you choose a periareolar incision, the scars will be hidden in a natural color demarcation of the skin at the edge of the areola. If an inframammary incision is chosen, the scar may be noticeable in the fold beneath the breast when the breast is elevated. Transumbilical incisions are made in the belly button and do not cause visible scars as they are hidden in the fold of the bellybutton.
What kind of breast implants feel better?
The new silicone cohesive gel implants resemble the feel of natural breast tissue more closely than saline implants; especially in thin patients without a lot of breast tissue and fat to camouflage the feel of the implant.
How are silicone implants filled?
Unlike saline implants that are inserted in the body empty and filled after insertion; silicone breast implants are pre-filled with a cohesive gel material from the manufacturer.
How are saline implants filled?
Saline implants are inserted into the body in a rolled up fashion similar to a cigar and then filled with a sterile salt water solution through a removable fill tube. The tube is then removed and a self-sealing valve prevents the fluid from leaking out.
What is the difference between saline and silicone breast implants?
Both saline and silicone implants have the same basic structure: an outer inert silicone shell. The difference is that saline implants are filled with a sterile salt water solution that is the same as that used for intravenous fluid replenishment, Silicone breast implants are pre-filled at the factory with a cohesive silicone gel material.

The Injectable fillers are temporary- lasting up to a year while the lip implant is permanent (but can be surgically removed). Fat can be injected into the lips also but requires liposuction to obtain the fat to be injected.
What do you do with the breast tissue behind the nipple that is left after male breast reduction surgery?
After liposuction for gynecomastia or male breast reduction surgery, there is often a small amount of hard breast tissue directly behind the nipple. This extra breast tissue that couldn’t be removed by the liposuction cannula can be directly excised by making a small incision on the bottom edge of the areola.
Can I breast feed after a breast reduction?
Since many of the ducts of the breast are not cut during the breast reduction surgical procedure, you will continue to make milk and could breastfeed after the surgery. However, the volume of the milk you produce will be diminished and therefore it is probably wise to supplement with bottled formula.

How do wounds heal?
- The first stage is called the inflammatory stage. Although it may appear that very little it happening during this stage in fact, it is very active. This is the time the body is producing collagen to strengthen the wound.
- The second phase, called the metabolic phase, lasts several weeks. During this phase the body produces thicker and stronger scar tissue than is ultimately needed to heal.
- The last and final stage of wound healing is called remodeling. It is during this phase that the body breaks down and remodels scar tissues, and decides how much scar will remain.
These three stages have a clinical significance to the patient. During the inflammation phase, the scar is not very strong. Redness of the wound and scar lasts throughout the metabolic phase. The redness does not fade for at least six to eight weeks after surgery or injury. It is at this stage that the scar is increasing in strength.
Of course, not everyone heals from wounds at the same rate. Many factors affect wound healing, including diabetes, HIV, vitamin deficiencies, and smoking. Some patients have a persistently heavy scar called hypertrophic scar or keloid. An unusually large or unsightly scar may be modified by later scar revision surgery.
A wound is not truly healed for at least 1 to 1½ years. It is rare to revise a surgical scar at least until this much time has passed.
Can I have cosmetic surgery if I smoke?
Smoking and nicotine patches can be especially damaging in surgeries like tummy tucks or facelifts, in which the skin is partially separated from its underlying blood supply and pulled tight under tension. It is mandatory to stop smoking at least two months before these procedures are performed.
You may be tempted to tell your doctor, “Oh, I never smoke,” when you actually have not been able to quit. Lying to your doctor is never a good idea, and in this case it can be a disaster. The effects of smoking on surgical recovery are very serious and deceiving your surgeon will only make things worse.

When is Botox therapy used?
Botox is designed to reduce horizontal lines of the forehead, frown lines, crows’ feet around the eyes, vertical lip lines, as well as the “number 11s” between your eyes. The side effects are usually confined to the area where the Botox is injected. There maybe minor discomfort as well as a possibly of a small bruise.
The action of Botox starts in approximately 24 hours and may take up to a week to work; the full effect is seen in approximately two weeks’ time.
It is okay to have a small amount of movement as long as the wrinkles lines are diminished. The objective of the treatment is to improve the appearance of the lines and not necessary paralyze the musculature. If an improvement can be obtained while retaining some minor muscle function that leaves you with some expression, that is often preferable than the “statue” look that is obtained with complete paralysis.
