The nose is the most central and the most prominent part of the face. More than any other anatomical feature, the nose contributes to facial balance and facial harmony. If the nose is even slightly out of proportion—relative to the rest of the face and other major features—then one’s self-image and confidence can suffer dramatically. A newly reshaped nose has the potential to alter your facial appearance dramatically.
Nothing has a greater impact on how a person looks than the size and shape of his or her nose. Because the nose is the most defining characteristic of one’s face, a slight alteration significantly improves one’s appearance as well as self-confidence.
Rhinoplasty is one of the most common facial cosmetic surgery procedures requested in America. Most surgeons agree rhinoplasty is a surgical art form and without a doubt one of the most complex operations in plastic surgery. Since no two noses are the same, it is of extreme importance that a surgeon have extensive knowledge and experience in all aspects of nasal surgery. Only this can ensure that the rhinoplasty performed is designed to suit the specific needs of each patient individually.

Problems in the facial structure that unbalance facial harmony include a small chin and lower jaw, weak cheeks, or mid-facial bone structure that contributes to unbalanced nasal facial proportions. In these cases, surgery of the nose may be combined with the placement of facial implants to impart symmetry to the face.
Surgery of the nose is best performed when facial growth is completed. This begins around age 15 for girls and age 16 for boys. Dr. Kole’s philosophy is to create a nose that fits the patient’s face and remains in proportion with the other facial structures. Every nose must be individualized and in balance with other facial features, and breathing through the nose must never be compromised or jeopardized as part of the operation. Dr. Kole’s rhinoplasty techniques produce individualized results that are subtle and natural appearing. We focus on strengthening and restructuring the cartilage framework of the nose to correct abnormalities. The result is an overall natural-appearing nose without nasal breathing problems.
Open and closed rhinoplasty. To create a naturally balanced appearance, the rhinoplasty procedure can be performed in either of two manners. In open approach rhinoplasty, a small incision is made between the nostrils, to elevate the entire skin off the cartilage framework. The alternative, closed approach rhinoplasty, does not make an incision across the nostrils. The choice of open or closed procedure is mostly dependent on the anatomy of the patient’s nose, as determined during pre-surgery consultation. Dr. Kole is trained in both the open and closed approach.
Using the open approach, the cartilage framework is directly visualized and is able to be sutured together and strengthened with cartilage grafts if necessary. The closed approach has a less certain postoperative healing course; Dr. Kole believes the closed approach results in higher revision rates.
Rhinoplasty can be performed with local or general anesthesia. The determining factor for anesthesia is whether the nose needs to be fractured or the septum needs to be straightened. In these cases, rhinoplasty is also performed under general anesthesia provided by our board-certified anesthesiologists in our surgical center due to the fact that bleeding can be more easily controlled and visualized.
A primary rhinoplasty procedure usually takes about one and a half to two hours’ time. Revision rhinoplasty—a secondary nose surgery when the patient is not pleased with the results of the original rhinoplasty—will take significantly longer; up to two and a half to three and a half hours is common. Following the surgery, packing is usually not placed inside the nostrils except in cases when the septum is straightened; sometimes a small silicone splint is placed in lieu of packing to allow the patient to breathe better in the immediate postoperative period. In all cases, a protective metal and foam-backed nasal splint is placed to avoid trauma to the area. That splint must stay in place for several days.
Nasal stuffiness is common for the majority of patients after surgery. Pain, however, is relatively minimal. Dr. Kole will provide proper follow-up care on a regular schedule. When the nasal splint is removed a couple of days after surgery, most patients see some visible signs of their rhinoplasty immediately. It should be noted that final results are seen only after the swelling has completely subsided, up to six months later.
Experience counts. Dr. Kole’s most frequently performed procedures are rhinoplasty and revision rhinoplasty (a secondary nose surgery when the patient is not pleased with the results of her original rhinoplasty).
